出口商声明信EXPORTER STATEMENT LETTER印尼使馆认证

印尼的一个客商让我们公司出一份印尼大使馆认证的EXPORTER STATEMENT LETTER，给他们做产品注册备案，这个文件要怎么弄呢，你们有做过的样本吗，有格式的吗？
——你好，印尼的进口比较复杂，他们在进口之前需要先在当地机构先申请进口申请，申请通过之后才可以进口货物，在申请进口许可证的时候就需要用到这份文件，EXPORTER STATEMENT LETTER主要目的就是用于进口商做产品备案注册，有固定的格式，可以向印尼进口商要取，或者也可以联系我司要取，然后按照自己出货的资料以及自己的公司资料将格式补充完整给印尼确认，确认没问题就可以安排做认证了；
所需资料：EXPORTER STATEMENT LETTER文件一式两份盖章
营业执照复印件
委托书一份

We are ready to held responasible and receive again the exported waste if it has been proven not in accordance to the point 1  (1.A Until 1.D) above.
